Modern medico-legal evaluation of Neonatal Cerebral Palsy (NCP) demands recognition of Neonatal Hypoxic Ischaemic Encephalopathy (NHIE) as an indispensable proof of peri-partum fetal hypoxia. Without proven NHIE, modern jurisprudence should not even consider obstetric/paediatric liability of causative peri-partum hypoxia of negligence on the grounds of medically responsible fetal/newborn hypoxia leading to Cerebral Palsy. Peripartum hypoxia, which comprises intra-partum hypoxia may result in damage which may with varying degrees of time manifest itself with epilepsy, neuro-developmental delay, cognitive impairment or Cerebral Palsy and the full damage may not be completely assessable before 3-4 years of age. Court cases alleging obstetric/ neonatologist liability may not commence for a number of years, at times even decades after he birth. In the case of Cerebral Palsy, it must be borne in mind that peri-partum hypoxia is not, nowadays, considered as causative in more than 9% of cases, whereas 91% may be due to premature birth, other complications or of undetermined aetiology. Since in many cases, CP litigation may reach Court without screening as to what may be liable from peripartum hypoxia or otherwise, it is crucial that in the hours or days post-birth, all relevant investigations are performed, stored safely and be accessible if and when necessary - Lupine Publishers

The peak spectral wavelength of all the devices tested had a peak centering around 450±10 nm, in billirubin absorption spectrum. LEDs achieved a higher maximum irradiance compared to CFL units across all the heights tested. CFL and Array LEDs showed a better mean irradiance compared to Spot/Multispot LEDs. CFL and Array LEDs demonstrated the best footprint whereas the Spot and Multispot LED units had a narrow footprint (approximately half or less of CFL or Array LEDs).Tilt angles influenced the mean irradiance. Most of the units showed a decrease in mean irradiance with tilting except for one Array LED namely Brilliance Classic. CFLs showed an inconsistent mean irradiance when tested at different voltage inputs LEDs had a consistent irradiance in spite of voltage fluctuations. LEDs had minimal irradiance decay of irradiance compared to CFLs. Both CFL and LED units showed increasing temperature output over time, which is more pronounced in CFL units - Lupine Publishers.

Although, arterial tortuosity is observed mostly in the head and neck vessels, it can occur other vessels as well. The clinical findings for musculoskelatal system involvement can be arachnodactyly, joint laxity, pectus deformities, scoliosis, dolichosternomelia, talipes equinovarus, comptodactyly, and cervical spine instability. Cardiovascular findings can be aortic diltation and disection, bicuspid aortic valve, and congenital heart disease such as atrial septal defect, patent ductus arteriosis. Central nervous system anomalies such as Chiari malformation and hydrocephalus can be observed. However, the life treatining complications that determine the prognosis are mostly cardiovascular system related findings like aortic root aneurysms, arterial tortuosity, aneurysyms of other vessels, patent ductus arteriosis, and atrial septal defect - Lupine Publishers.

Psychological and common diseases of children - Lupine Publishers


We report on a spontaneously delivered, female neonate, born in week 35+1 of gestation, weighing 2230g. It was presented pale, hypotonic, without respiratory movement and a heart rate below 100 bpm. Therapeutic treatment started with bag-maskventilation, followed by the installation of a pharyngeal tube and CPAP-ventilation for a total of six hours applying a PEEP of 6 millibar. The X-ray displayeda pneumomediastinum, showing elevation of both thymus lobes in the sense of an angel-wing sign (also known as spinnaker-sail sign) [1-3] (Figure 1). Ending CPAP ventilation, treatment continued vianasal cannula without oxygen supply for one more day, resulting in a stable respiratory status. The radiographic follow-up presented a noticeable regression of the pneumomediastinum (Figures 2 & 3 = 6th day of life; part C = 13th day of life), which made invasive measures redundant - Lupine Publishers

Acute scrotal conditions are common in children. It presents with scrotal pain, swelling, redness in the affected hemiscrotum. The true cause is difficult to determine. There are myriad etiologies for this syndrome and these are torsion of the testis (TT), torsion of the testicular appendix (TTA), epididymo-orchitis (EO) and strangulated inguinoscrotal hernia (SIH) [1-3]. General belief is that EO is rare in children and is associated with structural anomalies of the urinary tract [4-6]. The aim of this study is to review the various causes of pediatric acute scrotal conditions and to assess management of these patients under the light of relevant literature. Every boy with acute onset scrotal pain and swelling requires immediate evaluation. Commonest causes of acute scrotum in children are TT, EO, TTA [1-3,7]. Various incidences have been reported regarding the etiology of pediatric acute scrotum [1-3]. True incidence of these causes in acute scrotum is unclear, EO is thought to be uncommon [8] - Lupine Publishers.
